Prior to your appointment

Please fill out the MedXpress form. This is now a requirement by the FAA. After you make your appointment simply click on this link: MedXpress. Once you have completed the MedXpress form keep your confirmation number which will be at the bottom of the first page.

1. Email the confirmation number to: info@myflightsurgeon.com

2. Please bring the confirmation number with you to your appointment.
